• Full Name
• Full Address (Street Number and Name, City/Town, Province/State, Postal/Zip Code, Country)
• E-mail Address
• Phone Number
• Purchased Product (model name)
• Copy of Sales Receipt / Proof of Purchase showing date and the retailer this product was purchased from (please attach file)
• Picture(s) of Manufacturer's Plate* or Serial Numbers* (please attach file/s)
